Comments:
An announced monitoring inspection was conducted between 11:00am and 2:00pm. At the time of the tour there were 82 children in care with 14 staff present. Preschool children were observed during nap, school age children were watching a movie and infants were observed napping cribs, and playing on the floor with staff. The director reported there were no children in care with a diagnosed food allergy.

Standard #: 22VAC40-185-340-A
Description: Based on observation, interview and a record review the center did not ensure the care, protection and guidance when supervising children.
Evidence:
In the infant room a 9-month old child #1 was observed face-down on a nursing pillow, while staff in the room were attending to other Infants. The director confirmed staff did not place the child in the pillow for tummy time, rather, child #1 crawled to the nursing pillow on her own. According to the pillow's manufacturer usage instructions, which can be found on the product's website, there is a list of "Possible Suffocation Hazards." Under this list it states,
"Do not allow baby to lie face down" on the pillow.

Description: Based on observation the center did not ensure there was at least 12 inches of space between occupied cots.
Evidence:
In the preschool room there were 2 children napping on cots. There was about 8inches of space between occupied cots. Twelve inches is required.

Plan of Correction: Staff member is supervising children on the floor will ensure babies do not lay face down on the baby pillows.

Description: Based on observation the center did not ensure there was at least 12 inches of space between occupied cots.
Evidence:
In the preschool room there were 2 children napping on cots. There was about 8inches of space between occupied cots. Twelve inches is required.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will talk with the staff member and remind her that each cot should be at least 12 inches apart.
