An unannounced Renewal Inspection was conducted on August 13, 2019 from approximately 12:15 pm until 2:20 pm. Upon entrance there were 51 children and 4 staff present with children. The record review sample consisted of 6 children's records and the required documentation on site for 4 staff. Children were observed eating lunch and playing in centers Legos, cards, board games, blocks and coloring. Six medications were reviewed. The areas of noncompliance are noted on the violation notice and were confirmed with the Center Director during the exit interview. Standard #: 22VAC40-185-60-A Description: Based on review of 6 children' records and staff interviews, the licensee did not ensure that all children's records contained all required information.
Evidence: The record for child #3 did not include the employment phone number for one parent listed and the record for child #5 did not include the employment phone numbers for both parents listed and the city, state and zip code for both emergency contacts listed. The missing information was verified by the center director.Plan of Correction: The center director will reach out to parents to get the information and will double checks records prior to child entering program.
