An unannounced inspection on 07/30/19 in response to a self report regarding allegations in the area of supervision. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. As a result a violation was cited and can be found on the violation notice.

Standard #: 22VAC40-185-340-A Description: Based on a self report made by the program director, interviews, observations of the program and a record review, the center did not ensure the care, protection and guidance of children when staff are supervising. Evidence: On Wednesday 07/24/19 at approximately 3:00pm, staff #1 reported taking 3 month-old child #1 to the diapering area to change a wet bib. Staff #1 placed child #1 on the counter top and realized child #1's basket was on the other side of the diapering area. Based on Staff #1's written statement she "took one step towards the basket and looked away. When I looked back, Child #1 was on the floor on her back." Two other staff members who were in the room at the time of the incident, reported seeing child #1 on her back, on the floor and crying. The distance from the counter top to the floor was about 36 inches high. Plan of Correction: The following corrective action was provided by the Program Director during the exit interview: Staff #1 was placed on administrative leave. The director called the Infant -Toddler specialist to evaluate the infant room and program. The director is reviewing policies and evaluating if any changes need to be made to ensure something like this doesn't happen again.
