Comments:
The licensing inspector conducted an unannounced monitoring inspection on 07/30/2019 from 10:10am to 3:30pm. The children were observed playing on the playground, participating in water play, playing in centers, eating lunch and preparing for nap. A formal observation was completed with the twos and toddler classrooms. The menu was posted. Lunch consisted of sausage, biscuits, apple sauce, mixed vegetables and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Eight children?s records and six employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/29/2019 Last shelter-in-place drill: 09/18/2018 Last fire inspection: 02/26/2019 Last health inspection: 06/28/2019 Today, the following child to staff ratios were observed: Infants A - 7:2 Infants B - 7:2 Toddlers ? 9:2 Twos - 10:2 Preschool ? 18:2 Pre - Kindergarten- 20:2 School Age - 13:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was sharp and splintering wood located on the center's playground. 2. The picnic tables and benches were rusted and had peeling paint. 3. The inspector observed chipped and peeling paint in the twos, preschool, pre-k and school age classrooms.

Plan of Correction: - Emergency/Urgent work order(s) have been put in for all of the equipment (sharp splintering wood, picnic tables, peeling paint) on August 1, 2019. - All work will be completed in 30 days. - Monthly Center Safety Checklist will be used moving forward

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets had protective covers that are of a size that cannot be swallowed by children. Evidence: 1. The preschool classroom had on uncovered outlet out of the reach of children in care. 2. The pre-k classroom had two uncovered outlets out of the reach of children in care.

Plan of Correction: - Outlets in both the preschool and PreK classrooms were immediately covered on 07/31/19 - Staff was retrained on why outlets need to be covered on 07/31/19

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that the preschool/school age playground did not have sufficient resilient surfacing underneath and surrounding the climbing play equipment. Evidence: There were two multicolored composite climbing structures located on the preschool/school age playground. Directly underneath and surrounding the entire structure was wooden mulch as well as decomposed wooden mulch which has turned to mud/dirt. The inspector measured several areas around the structure. The wooden mulch measured approximately 3 to 6.5 inches in depth in the use zones of the tall yellow slide and blue slide. The use zone for both of the slides required six inches of resilient surfacing.

Plan of Correction: Emergency/Urgent work orders were put in for mulch on preschool/PreK playground on 08/01/2019.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not ensure that the staff and children's hands were washed after diapering, and before eating snack. Evidence: The inspector observed the following: 1. The two year old teacher was observed changing five children's diapers consecutively. The teacher did not wash their hands before or after diapering each child, nor the children's hands after diapering. 2. The toddler teacher did not wash three toddler's hands with soap and running water after diapering. 3. The toddler teachers did not wash the toddlers hands before snack.

Plan of Correction: Toddler and Two's teachers were retrained on proper hand washing techniques for children and staff during meal times on and diapering on August 1, 2019. Management will monitor classrooms daily at meals and diaper changes to ensure proper procedures

Standard #: 22VAC40-185-550-D
Description: Based on record review and interviews, the center did not ensure to practice evacuation drills monthly. Evidence: The center did not have record of an evacuation drill in the month of April. The center's representative stated that the center missed April's evacuation drill and the subsequent drill was practiced on May 30, 2019.

Plan of Correction: Document has been created to ensure monthly fire drills are being completed . Monthly reminder has been set to calendar

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure that four of six staff records contained documentation of central registry results by the end of the 30th day of employment. Evidence: 1. The record of staff #1 (start date: 4/12/2019) had documentation of central registry results dated 5/20/2019. 2. The record of staff #3 (start date: 4/13/2019) had documentation of central registry results dated 6/05/2019. 3. The record of staff #4 (start date: 4/13/2019) had documentation of central registry results dated 5/24/2019. 4. The record of staff #6 (start date: 4/12/2019) had documentation of central registry results dated 6/03/2019.

Plan of Correction: - Spreadsheet has been created to ensure documentation is noted when inquires have to be made after 30 days of no response. - Center Director will set alerts on iPad to make sure check-ins happen within 30 days of sending out original form.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not ensure that one of six staff records had documentation of fingerprint results prior to the first day of employment. Evidence: The record of staff #1 (start date: 4/12/2019) had documentation of fingerprint results dated 4/26/2019.

Plan of Correction: Teacher #1 was not clocked in until 4/30/19, fingerprints came back on 4/26/19.
