Comments:
The licensing inspector conducted an unannounced renewal inspection on 08/09/2019 from 9:45am to 4:05pm. The children were observed participating in story time, playing in centers, reading books, eating lunch and preparing for nap time. A formal observation was completed with the toddlers and school age classrooms. Lunch consisted of beef and cheese tacos, applesauce, carrots and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini bus. Eight children?s records and six employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan, which was updated 6/28/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/30/2019 Last shelter-in-place drill: 05/06/2019 Last fire inspection: 03/28/2019 Last health inspection: 07/17/2019 Today, the following child to staff ratios were observed: Infants - 7:2 Toddlers ? 4:2 Twos - 16:2 Preschool ? 18:2 Pre-Kindergarten - 12:2 School Age - 19:2 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. The Department has not yet received a complete renewal application from the center. A complete renewal application must be received by the Department prior to expiration of the license on October 1, 2019 or the license cannot be renewed. Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The inspector observed a water hose hanging on the playground fence presenting an entanglement hazard.

Plan of Correction: Corrected during the inspection. I will go over this with the staff.

Standard #: 22VAC40-185-330-B
Description: Based on observation and interviews, the center did not ensure that where play equipment is provided, resilient surfacing complied with minimum safety standards described in the American Society for Testing and Materials. Evidence: The slide located on the infant/toddler playground did not have enough resilient surfacing throughout the entire fall zone. There must be at least six feet of resilient surfacing from the base of the slide. The center had about three feet of resilient surfacing from the base of the slide. The inspector observed a toddler child playing on the slide. Interviews with the toddler teachers revealed that the toddler children regularly play on the slide.

Plan of Correction: We will close the slide until it is redesigned.
