Comments:
An unannounced non-mandated inspection was conducted on 08/12/2019 from 9:50 A.M.-1:15 P.M. There were nine children present with three staff and one administrator present. Five children records, four staff records, first aid and emergency supplies, evacuation drill, outdoor play area, over the counter skin products and required documentation, four injury reports, and required posted information were reviewed. Children were observed during transition time, handwashing, diapering, free choice activities, calendar time, counting activities, storytime, lunch time, and preparation for naptime. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children files, the center failed to ensure that all required information was obtained and in the children files. Evidence: 1. Child #2 and Child #3's file did not contain a complete address for one emergency contact, and did not have any address listed for the second emergency contact. 2. Staff verified that the files for Child #2 and Child #3 were missing complete addresses for emergency contacts.

Plan of Correction: Administration will contact the parents this afternoon at pick-up and obtain the complete address for the emergency contacts listed.

Standard #: 22VAC40-185-520-A
Description: Based on review of over the counter skin products, the center failed to ensure that over the counter skins products will not be kept or used beyond the expiration date. Evidence: 1. Child #4 had a bottle of Ultra Sheer Sunscreen that expired in September 2017. 2. Child #5 had a roll on container of Honest Mineral Sunscreen that expired in January 2019. 3. Staff verified the expiration dates of the sunscreen.

Plan of Correction: Administration will contact the parents at pick up this afternoon, and return the expired sunscreen. Administration will also review the standards pertaining to over the counter skin products.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that food brought in from home was dated. Evidence: 1. Nine out of nine lunchboxes brought in from home were not dated. 2. Staff verified that the lunchboxes were not dated.

Plan of Correction: Administration will review the policy with staff regarding food brought in from home, and will immediately start dating all lunchboxes or food brought in from home.
