Comments:
An unannounced monitoring inspection was conducted on 8/16/19 from 12:25pm until 1:45pm. At the time of the tour, there were 46 children in care with 7 direct care staff and 3 administrative staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, reading and resting quietly during nap time. Hand washing and restroom procedures were also observed. Medication, the emergency preparedness plan, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of background checks is identified on the violation notice and was discussed with the center owner during the exit interview.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review and interview, it was determined that the licensee did not ensure that all employees hired prior to January 22, 2018 have a completed fingerprint based national criminal history record check by September 30, 2018. Evidence: 1. The record for staff 1 does not contain documentation of a completed fingerprint based national criminal history record check. There was a completed Virginia criminal record check dated 9/19/16. Staff 1 has a documented date of hire of 8/26/07 and was working on the day of the inspection. 2. Center administration confirmed that a fingerprint based background check was not completed for staff 1.

Plan of Correction: Administration will ensure a fingerprint appointment for staff 1 is scheduled today. Once the results are received, they will be forwarded to the licensing inspector. Staff 1 will not work at the center until the fingerprint results are received.
