Comments:
An unannounced monitoring inspection was conducted on 8/14/19 from 9:40am - 12:25pm. During the inspection there were 78 children ages five months old through nine years old in care with 15 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for eight children and eight staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of eight staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: 1. The record for staff #4 (date of hire 7/11/19) c did not contain documentation of a negative tuberculosis screening. 2. Staff #3 reviewed the record for staff #4, confirmed that the documentation of a negative tuberculosis screening had not been received

Plan of Correction: The facility responded: Staff #4 has returned to her teaching position with Norfolk Public Schools for the school year, but we will get a copy of her TB test to keep in her file.

Standard #: 22VAC40-185-60-A
Description: Based on a review of eight children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not contain the name, address and phone number for the second emergency contact. 2. Staff #3 confirmed that the records for child #1 did not contain all of the required information.

Plan of Correction: The facility responded: A 2nd emergency contact has been added to child #1's file.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of eight staff records and interview, it was determined that the licensee did not ensure that all employees hired prior to January 22, 2018 have a completed national criminal history record check (finger printing) by September 30, 2018. Evidence: 1. The record for staff #1 (date of hire 7/18/17), working during the inspection, did not contain documentation of a completed national criminal history record check (finger printing). There was a completed criminal record check that was dated 9/15/17, however it was not the background check required by Licensing. 2. The record for staff #2 (date of hire 6/19/17), working during the inspection, did not contain documentation of a completed national criminal history record check (finger printing). There was a completed criminal record check that was dated 8/23/17, however it was not the background check required by Licensing. 3. The record for staff #3 (date of hire 7/14/99), working during the inspection, did not contain documentation of a completed national criminal history record check (finger printing). There was a completed criminal record check that was dated 12/19/16, however it was not the background check required by Licensing. 4.The record for staff #5 did not contain documentation of a completed national criminal history record check (finger printing). There was a completed criminal record check that was dated 12/19/16, however it was not the background check required by Licensing. 5. The record for staff #6, working during the inspection, did not contain documentation of a completed national criminal history record check (finger printing). There was a completed criminal record check that was dated 12/9/16, however it was not the background check required by Licensing. 6. Staff #3 and staff #6 reviewed the records, and confirmed that none of the staff listed above had the results of the national criminal history record check (finger printing) required by Licensing by September 30, 2018.

Plan of Correction: The facility responded: All staff employed prior to 9/1/2017 were fingerprinted by the VA State Police and cleared. Their fingerprints will be rerun by a Fieldprint Authorized facility within the next 5 days and reports completed and in the employee's file by 9/1/2019.
