Comments:
An unannounced inspection was conducted today, August 19, 2019 in response to a complaint received by The Department of Social Services, Licensing on July 24, 2019 concerning physical plant. The inspection was initiated at 9:15am and concluded at 11:20am. Staff interviews were conducted, observations were made and documentation reviewed. Based on the information gathered during the investigation, the preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Though not a part of the original complaint, there were additional violations found during the inspection. * See the violation notice for more details. Standard #: 22VAC40-185-70-A
Complaint related: No
Description: Based on review of one staff record, the center did not obtain all required documentation. Evidence: The record of staff #2, identified as a program leader, did not contain written documentation of education required by the job position.

Plan of Correction: Per the owner: The staff member has the qualifications but has not received her transcript. She did request it last week.

Complaint related: Yes
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: Four containers of cleaning substances were observed unlock on a wall shelf, approximately six feet from the floor, in the office. Children must walk through the office space to get to the school age classroom.

Plan of Correction: Per the owner: I will remove the cleaning substances and put them in a locked cabinet.

Description: Based on observation and interview, the center did not ensure during the stated hours of operation, there was on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: 1. On August 19, 2019, staff #1 stated the school age children were down the street at the Washington Avenue Park playground with one staff member. 2. Thirteen children ages three years through eleven years were observed at the Washington Avenue Park playground, approximately one and a half blocks from the center with one staff. The playground is not adjacent to the center.

Plan of Correction: Per the owner: I will have a second staff go with all groups to the playground.

Description: Based on interview and observation, the center did not ensure a first aid kit was accessible to outdoor play areas; on field trips; and wherever children are in care. Evidence: 1. Thirteen children and one staff were observed at the public playground approximately one and a half blocks from the center without a first aid kit. 2. Staff #2 stated she did not bring a first aid kit with her to the playground.

Plan of Correction: Per the owner: I will have staff take a first aid kit with them whenever they are off site with children.
