An unannounced monitoring inspection was completed on 08/19/2019 from 3:00 P.M.-5:30 P.M. At the time of the inspection 24 children were in care with one staff, and one site manager. Five children records, emergency evacuations, first aid kits, emergency supplies, emergency plan, injury prevention plan, playground safety plan, fire inspection, health inspection, and posted required information were reviewed. Staff records were reviewed off-site on 08/19/2019 from 11:15 A.M.-1:30 P.M. Children were observed during arrival, free choice activities that included games, books, art activities, handwashing, snack time, homework time, behavioral guidance, gross motor time in the gym, and dismissal. Standard #: 22VAC40-185-340-D Description: Based on observation, the center failed to ensure that at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Staff #1 was alone with a group of children between the ages of 5-8 years of age for approximately 20 minutes in the gym. Staff #1 does not qualify as a program leader or a program director. 2. Staff #1 was alone with a group of children between the ages of 9-11 years of age for approximately 20 minutes in the gym. Staff #1 does not qualify as a program leader or a program director. 3. Staff #2 verified that Staff #1 does not qualify as a program leader or program director. Plan of Correction: Administration and staff will review the daily schedule to ensure that a program lead is always present with a group of children.
