Comments:
An unannounced mandated Monitoring inspection was conducted today. There were 13 children present with 3 staff. Children were observed playing with musical instruments, singing songs, cleaning up, having circle time, toileting/handwashing, having lunch (pizza, chicken nuggets, fruit cup and milk), and napping. Reviewed 4 children's files, 5 staff files, attendance, emergency evacuation log, first aid kit, fire certificate, menus, and physical plant. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, each staff member and individual from an independent contractor did not submit documentation of a negative tuberculosis screening within 21 days of employment. Evidence: Staff#5 (start date 7/26/19) and Contractor#1(start date approx. 6/24/2019) did not have documentation of a negative TB test on file.

Plan of Correction: I will have them get them done.

Standard #: 22VAC40-185-40-E
Description: Based on record review, the center did not ensure that the center's activities, services, and facilities were maintained in compliance with the terms of the current license issued by the department. Evidence: Child#2 (start date 12/14/18) was under 2 1/2 years old on the first day of care. Center's License is currently for ages 2 1/2 years and above only.

Plan of Correction: We will not enroll children who are under two and a half years of age.

Standard #: 22VAC40-185-240-A
Description: Based on staff record review, staff did not receive the recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse training by the end of their first day of assuming job responsibilities. Evidence: Staff#5 (start date 7/26/19) did not have documentation of completing Child Abuse and Neglect training.

Plan of Correction: I will make sure she completes the training or find the document.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the resilient surfacing on the outdoor playground does not comply with minimum safety standards of 6 inches in depth. Evidence: The resilient material (mulch) is less than 6 inches deep under fall zones and slide exit.

Plan of Correction: We will be purchasing more mulch.

Standard #: 22VAC40-185-560-F
Description: Based on observation, center meals did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The lunch served by the center during this inspection (pizza, chicken nuggets, fruit cup and milk) did not contain a vegetable component as required. Further review of menu revealed that every Friday in the month of August was missing a vegetable component.

Plan of Correction: We will be adding a vegetable to all meals.
