Comments:
An unannounced monitoring inspection was conducted today from 10:45am-1:50pm. There were 88 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant, 5 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, having storytime, transitioning from waterplay. Diapering, hand-washing, and bottle-feeding procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: A container of chemical floor cleaner was found in an unlocked storage closet in the children's bathroom area.

Standard #: 22VAC40-185-500-A
Description: Based on observation, a staff member did not ensure that a child's hands were washed with soap and running water after toileting. Evidence: Staff E changed Child #2's diaper and did not wash the child's hands before allowing to child to return to the group activity.

Description: Based on record review, 1 of 10 injury reports did not contain all required information. Evidence: One injury report did not include documentation of the method, date, and time of parent notification.
