Technical Assistance:
Technical assistance was provided regarding what is required in the injury prevention plan and emergency contact annual updates.

Comments:
An unannounced monitoring inspection was conducted from 3:00pm-7:00pm. At the time of inspection, 64 children were in care with 10 staff members present. Children were observed during snack time, engaged in group activities, arriving from school, singing and reading. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of 5 staff and 6 children records, the physical space, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-40-J
Description: Based on interview with director, an injury prevention plan was not available for review. Evidence: There is no documentation of injury prevention plan.

Plan of Correction: Will update injury prevention plan after reviewing recent accident reports.

Standard #: 22VAC40-185-260-A
Description: Based on review of emergency records, there is no documentation of an updated annual fire inspection report from the appropriate fire official. Evidence: Current fire inspection report expired on 04/30/19.

Plan of Correction: I will contact Fire Marshal on Monday 8/26/19 and request an updated inspection.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation, 1 out of 7 classrooms was out of ratio. Staff-to-children requirements for children from birth to the age of 16 months: one staff member for every four children. Evidence: In the toddler room, there was 1 staff member with 5 children, child #4 was under the age of 16 months.

Plan of Correction: When transitioning a child, we will follow ratio of youngest child.

Description: Based on interview with staff member, the amount of time spent on theirs stomachs was not available for review. Evidence: In the infant room, there was no documentation of any tummy time available for review.

Plan of Correction: Will remind infant staff to document tummy time immediately after done.

Standard #: 22VAC40-185-420-E-3
Description: Based on review of 5 children's records, 5 records were missing annual updates of parental emergency contact information. Evidence: Children #1, 2, 3, 4, and 5 do not have documentation of annual updates of parental emergency contact information.

Plan of Correction: Have a new form ready to distribute to parents for updated emergency contact info.

Description: Based on observation, cots and rest mats were not identified for use by a specific child. Evidence: Cots and rest mats were not identified for use by a specific child.

Plan of Correction: Cots are numbered in PreK class already and will be numbered in other rooms.

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointment or cream were accessible to children. Evidence: In the 2 year old classroom, diaper cream was located in the bottom shelf making it accessible to children.

Plan of Correction: Will remove lotions from location and place where inaccessible.
