Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on Thursday, August 22, 2019 to review supplemental health and safety requirements. The inspector arrived at the center at 10:00 am and concluded the inspection at approximately 2:15 pm. The center's director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 38 children in the care of 8 staff. Upon the inspector's arrival, the children were engaged in a variety of activities. The theme of the week was the letter "B." The children in the Three's classroom were observed playing in centers, interacting with peers and staff, playing with blocks, dress-up, play-dough, and during an art activity led by the staff. The children in the Two's classroom were observed as they transitioned from the playground to their classroom. They were later observed during free-play, taking bathroom breaks, washing hands, eating lunch, and later during their designated rest period. The children in the Toddler's classroom were observed coloring at the table and going over number and animal themed flashcards. In the Infant classroom, the children were observed as they slept, while they were fed, doing tummy time, and staff were observed during diapering procedures. Staff in all classrooms were observed having positive interactions with the children in care. The classrooms are decorated with the children's artwork and are equipped with age and sate appropriate materials. The playground was inspected and found to be in compliance. Medication is not administered at this time and transportation is not provided. The first aid kit and non-emergency supplies were inspected and found complete. There are staff available with Daily Health Observation training, CPR and First Aid certified staff.

During the inspection, five children's records, five staff records, and background checks for the center's owners were reviewed.

Evacuation Drill: 07/03/2019; shelter-in-place drill: 09/17/2018; and lockdown drill for 2019 has been scheduled.

A licensing inspection was also conducted this date; the two inspections combined over all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records on 08/22/2019, the vendor did not ensure that three staff were evaluated by a health professional and issued a statement that staff is determined free of communicable tuberculosis (TB) within the required time frames.

Evidence: 1) The record for Staff #2, hired on 04/25/2019, contained a TB screening result that was dated 04/30/2019. 2) The record for Staff #4, hired on 03/25/2019, contained a TB screening result that was dated 03/26/2019. 3) The record for Staff #3, hired on 03/11/2019, contained a TB screening result that was dated 11/29/2018. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Already corrected. The director is aware of the change and will ensure TB results are submitted timely. Staff will not be able to start employment without the TB results.

Standard #: 22VAC40-665-580-B
Description: Based on a review of five staff records and interview on 08/22/2019, the vendor did not ensure that three staff completed the required Virginia Preservice Training for Child Care Staff within 90 days of employment.

Evidence: 1) The record for Staff #2, hired on 04/25/2019, did not contain documentation that the staff completed the Virginia Preservice Training for Child Care Staff. 2) The record for Staff #3, hired on 03/11/2019, did not contain documentation that the staff completed the Virginia Preservice Training for Child Care Staff. 3) During interview, the director reported that Staff #2 and Staff #3 completed the training, but the documentation was not available for review at the time of the inspection. 5) The record for Staff #4, hired on 03/25/2019, contained documentation that the Virginia Preservice Training for Child Care Staff was completed on 07/25/2019, exceeding the 90 day requirement.

Plan of Correction: Staff will be required to provide the training within the required time frame. Staff #2 and Staff #3 will be providing the director a copy of the training so it can be filed.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of five staff records on 08/22/2019, the vendor did not ensure that one staff had current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of the date of employment.

Evidence: 1) The record for Staff #4, hired on 03/25/2019, did not contain current CPR certification.

Plan of Correction: The director reported Staff #4 will be completing the in-person portion of the training on 08/31/2019. In the future, staff will complete the required CPR certification within the required deadline.

Standard #: 22VAC40-665-580-E-2
Description: Based on a review of five staff records on 08/22/2019, the vendor did not ensure that one staff had current first aid certification within 90 days of the date of employment.

Evidence: 1) The record for Staff #4, hired on 03/25/2019, did not contain current first aid certification.

Plan of Correction: The director reported Staff #4 will be completing the in-person portion of the training on 08/31/2019. In the future, staff will complete the required first aid certification within the required deadline.
