Description: Based on a review of staff records, the center failed to ensure that each staff record contained documentation of 2 or more references. Evidence: Staff #1's record contains only one references and staff #5's record contained only one reference.

Plan of Correction: I will have Executive Director contact additional references for Staff #1 and Staff #5 and place in their files.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that areas of the center inside will be maintained in a clean, safe and operable condition. Evidence: 1) Three ceiling tiles in the hallway are stained and appear to have water damage. 2) Paint is peeling off the wall in the main hallway

Plan of Correction: We will replace the ceiling tiles and patch the wall.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: There were two lunch boxes that were not labeled with the name and date.

Plan of Correction: We will remind parents to label lunch boxes.
