Comments:
An unannounced monitoring inspection was done today August 28, 2019 beginning at 1:45 PM and ending at 5:45 PM. There were 119 children in care today, including the school age children. The children were observed in all the classrooms involved in a variety of activities. They were reading books, completing homework, eating snacks, and engaging in free play. The outside area and all medications were reviewed. The inspector also reviewed a sample of ten children and ten staff files. This included newly enrolled children and hired staff. There were discussions about seasonal employees and record keeping. The violations and corrective action were discussed with the director today. Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:

The handrail located at the rear of the building had visible signs of rust and wear, posing as a potential risk for injury.

Plan of Correction: The handrail will be replaced.

Standard #: 22VAC40-185-280-B
Description: Based on observation and discussion with staff, the facility failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. There was an aerosol spray glass cleaner located on the bus near the entryway.
2. There was air freshener located in the entryway of the facility, located on the floor and accessible to the public.
3. The inspector spoke with staff and asked if staff knew the air freshener was located in the entryway and they stated that they were aware.

Plan of Correction: Items were removed.
