Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements, the center did not ensure where playground equipment is provided, resilient surfacing complied with minimum safety standards under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:

1. Three measurements for the depth of resilient surfacing at the foot of the slide on the preschool playground were taken consisting of one inch, one inch and one and a half inches.
2. The ground was visible in areas around the foot of the slide.

Plan of Correction: Per the Director: We will rack the resilient surfacing from the outside boarders where it is not required to the fall zone area around the equipment . If additional pea gravel is needed we will purchase and install it to ensure it meets the required depth and distance around the equipment.
