**Tuberculosis test or screening is required before coming into contact with children. Documentation must have been completed within the last 30 calendar days of the date of employment.

Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.
An unannounced Subsidy Health and Safety Inspection was conducted on August 26, 2019 to review supplemental health and safety requirements. A licensing inspection was also conducted on this date (see separate report for details). Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection. There were 58 school age children present with seven staff. All staff must have documentation of the mandatory VA Pre-service Training and current certification in first aid and CPR within 90 days of employment. Children with diagnosed food allergies must have an Allergy Action Plan completed by a physician. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. The last shelter in place drill was conducted 1/14/2019; the last lockdown drill was conducted 2/1/2019.

Evidence: there was no documentation to support that Staff 1 and Staff 2 have current certification in CPR. Plan of Correction: Staff 1 and Staff 2 will get documentation of current CPR training.
