An unannounced monitoring inspection was conducted to investigate a self reported related to forbidden action and behavioral guidance of the children incident at the center. The inspection was conducted with the director on May 17, 2019 beginning at 10:00 AM and ending at 12:20 PM. Three staff interviews were conducted. One child's records and two staff records were reviewed and determined to be in compliance. Video recording and center documents were reviewed. A classroom observation was conducted. Violations were issued as a result of this inspection and a corrective action plan had already been developed by the center prior to the investigation. Standard #: 22VAC40-185-410-1 Description: Based on a review of video recording, a staff committed a forbidden action by roughly handling a child in the infant room. Evidence: Staff A was observed harshly picking up an infant that had climbed on top of a table. The child was lifted from the table with such force that his legs swung forward and placed in the sitting position on the floor. Plan of Correction: Once the incident was reported to the director, the teacher was immediately placed on unpaid administrative leave. We then contacted CPS and Licensing to report the incident. The employee was terminated following our investigation. The Director maintained open communication with the parents of this classroom. One on one re-training was held with each employee in the classroom. The parents are pleased with the improvements.
