Comments:
An unannounced renewal inspection took place on September 4, 2019. There were 24 children in care from ages 3 years old to 5 years old with 7 staff providing care and supervision. The inspector reviewed 5 children's records on site. On 08/30/2019, the LI reviewed 5 staff records, policies and procedures, and confirmed board officer records at another site for this center. The inspector reviewed the only medication at the center. Children were observed during the following activities: story time, hand washing practices, preparation for nap-time, and lunch time. The inspector also reviewed emergency drill documentation, first aid kits, and emergency supplies. There were discussions about facility maintenance and playground maintenance. The inspection was conducted from approximately 12:00 PM to 2:45 PM. Findings were reviewed with staff at the conclusion. Description: Based on record review and discussion, the facility failed to ensure that each staff record included all elements required by the standards.

Evidence:

1. Staff (2) and (5) did not have an address listed for his/her emergency contact.

Plan of Correction: The facility has already stated that records are being corrected.
