- Technical Assistance:
An unannounced monitoring inspection was conducted on Thursday, August 29, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at the center at 2:15pm and concluded the inspection at approximately 5:15pm. The center?s director was present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 32 children in the direct care of 5 staff. Upon the inspector?s arrival, the preschool children were resting/napping. The school age children were observed in their designated classroom watching a movie and reading books. The children were later observed outside in their respective playground. They were observed during free-play and interacting with peer and staff. The staff were observed having positive interactions with the children. The playground areas were inspected. The classrooms are decorated with the children?s artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. Five children's records, five staff records were reviewed during today's inspection. Per the director, there are no medications on site at this time. The last documented evacuation drill was held on 07/31/2019 and one has been scheduled by 8/31/2019. The last shelter-in-place drill was held on 7/03/2019.

Standard #: 22VAC40-185-160-C Description: Based on a review of five staff records and interview on 08/29/2019, the center did not ensure that one staff submitted tuberculosis (TB) test results at least every two years from the date of the first initial screening or testing.
Evidence: 1) The record for Staff #5, hired on 03/10/2004, contained TB screening results that were dated 06/12/2017. The screening expired on 06/12/2019. 2) During interview, Staff #5 reported the TB screening results on file expired on 06/12/2019.
Plan of Correction: Per the Center: The staff will have the new results by the end of next week. In the future, management will monitor records and dates to ensure TB screening/test results are resubmitted timely.
Standard #: 22VAC40-185-60-A Description: Based on a review of five children's records and interview on 08/29/2019, the center did not ensure that one child's record contained the required information.
Evidence: 1) The record for Child #1, enrolled on 08/27/2019, did not contain addresses for the two emergency contacts listed. 2) During interview, a member of management reported the two addresses had not been provided to date. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: Per the Center: The information will be requested from the parent and will be filed by tomorrow. In the future, paperwork will be checked for completion before the child attends the center.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on a review of five staff records and interview on 08/29/2019, the center did not ensure that one staff obtained fingerprint results prior to hire.
Evidence: 1) Staff #1, hired on 07/26/2019, did not obtain fingerprint results prior to hire. The fingerprint results were dated 07/29/2019. 2) During interview, a member of management reported that Staff #1 was not placed in the classroom with children, but received orientation training on 07/26/2019, prior to the center obtaining the fingerprint check letter of eligibility.
Plan of Correction: Per the Center: In the future, staff will not be employed until the fingerprint results are returned.
