- Technical Assistance:
Consultation provided on maintaining accurate attendance, safe sleep to include not using soft bedding- filled blankets in cribs.
Consultation provided on the gap ( between the ground and fence) on the infant and the Montessori playgrounds- although they are not head or torso entrapments, a child could potentially get their foot stuck in these openings and twist their ankle so please enclose that openings to prevent a foot from getting entrapped.
Standard 570H states that prepared baby food not consumed during that feeding by an infant may be used by the same infant later in the same day , provided that the food is not served out of the baby jar and is dated and stored in the refrigerator; otherwise, it shall be discarded or returned to the parent at the end of the day. Formula or breast milk shall not remain unrefrigerated for more than two hours and may not be reheated.
An unannounced monitoring inspection was conducted today with the director, who was present with 19 children with 6 staff providing supervision. The Licensing Inspectors were able to observe the infants playing with bubbles outside on their playground enjoying the sunny day; the teacher reading a book to the toddlers; the children in the Montessori classroom coloring and the JR'K children painting. Six staff and children's files were reviewed today. One child had medication on site. Ten injury/accident reports were reviewed today. Both buses were inspected as well. Please submit your plan of correction by 9/3/19.

Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that the step on diaper container is used in a way that is not clean as the door handle to access the container is contaminated by staff hands while disposing of soiled diapers..
The step on container used for soiled diapers was located in a enclosed area where the staff would need to touch the door handle to open the door in order to dispose of soiled diapers. This potentially contaminates the door handle.
Plan of Correction: Corrected on-site during visit. Director moved the trashcan from the bathroom back to the area beside the changing table where it originally was located beside the changing table and the step on trashcan will remain in that location permanently. Also spoke to the teachers in the toddler classroom about not moving the trashcan back into the bathroom and reiterated the importance of trash can location in diaper changing rooms.
Standard #: 22VAC40-185-330-B Description: Based on observation, it was determined that there was insufficient resilient surfacing surrounding the composite play structure on the Montessori playground. The standard requires at least six inches of resilient surfacing.
There was one inch of mulch surrounding the play structure on the Montessori playground.
Plan of Correction: Immediately after the inspector left, the director reached out to maintenance and purchasing to let them know that the mulch was insufficient and needed to be replaced. Mulch was immediately ordered and the director will follow-up with maintenance for a delivery date of new mulch.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of six staff files and an interview with the director, it was determined that one staff did not have documentation of the out of state central registry search within 45 days.
The center could provide no documentation that they had received the results of or any information regarding the out of state central registry findings for staff A. Staff A was hired on 4/15/19 and although the background check was sent off 4/17/19 there was no information regarding the results or follow-up.
Plan of Correction: Employee has fingerprinting results, sworn statement, along with the central registry results. Complete packet was filled out for the out-of-state background check and was present in the file. Previous manager did not follow-up with results. New director re-sent the packet for the background check the day of the licensing visit and will follow-up with the state until the results are received.
