Comments:
An unannounced monitoring inspection was conducted from approximately 7:00am-8:35am on September 5, 2019. Upon arrival to the facility, there were three staff present with 20 school age children in the cafeteria. Children were observed arriving to the program, working with table top manipulatives and talking freely amongst themselves. Five children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor areas, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation and procedures, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Five medications were reviewed in the center on this date. Exit interview conducted with the Assistant Recreation Supervisor.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review, the licensee did not ensure that all required information for each staff member was kept at the center.

Evidence: The Assistant Recreation Supervisor confirmed that there was no documentation of Staff #3's name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency at the center.

Plan of Correction: Assistant Recreation Supervisor will go to the office and print out her form so that she can fill it out.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure that emergency evacuation and shelter-in-place maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Evidence: The Assistant Recreation Supervisor confirmed that there was not an evacuation and shelter-in-place map posted in the center.

Plan of Correction: The map was posted during the inspection.
