Comments:
A Subsidy Health and Safety inspection was conducted on 09/04/2019 to review supplemental health and safety requirements. There were 22 children, ages 4 years-12 years, and 4 staff present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 2 medications during the inspection. Children were observed in the following activities: arriving to the program, during homework time, having an afternoon snack and going to the gym to play. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed subsidy requirements, transportation and staff records with the staff. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the Violation Notice. Description: Based on document review and discussion with the staff, the vendor failed to ensure that emergency evacuation procedures shall contain procedures to address reuniting children with a parent or an authorized person designated by the parent to pick up the child.

Evidence: The emergency evacuation procedures did not contain procedures to address reuniting children with a parent or an authorized person.

Plan of Correction: The model form provided by VDSS will be used to meet the requirement.

Description: Based on document review and discussion with the staff, the vendor failed to ensure that shelter-in-place procedures shall contain procedures to address reuniting children with a parent or an authorized person designated by the parent to pick up the child.

Evidence: The shelter-in-place procedures did not contain procedures to address reuniting children with a parent or an authorized person.

Plan of Correction: The model form provided by VDSS will be used to meet the requirement.
