Comments:
An unannounced monitoring inspection was conducted on Friday, August 16, 2019 to determine the center's compliance with licensing standards The inspector arrived at 10:30 AM and concluded the inspection at 1:45 PM. The census for today's inspection consisted of 59 children. During the inspection, staff and children were observed coloring, exercising, playing on the playground, washing hands, eating lunch, art, cooking and playing computer games and napping. Children's equipment, learning materials, outdoor and indoor space, restrooms, first aid kit and the nonmedical emergency supplies and injury reports were reviewed. Five children's records and five staff records, annual fire and health inspections, fire drill, lockdown and shelter in place drill logs, daily attendance, and all required postings were reviewed. Medication is administered and medication, authorization forms and allergy plans were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. On the playground, the edging of the concrete area was elevated from the grassy/mulch play area creating a tripping hazard.
2. Trash/litter was observed on the playground and included three bags of trash, candy wrappers, paper towels and cups.
3. Classroom #3 was observed playing on the playground.
4. The fence surrounding the playground had a gap between the end of the fence and the building. The licensing inspector observed a child squeezing through the gap. A four to five foot drop is located behind the fence.
5. A crushed beer can was on the ground outside of the fence accessible to children causing unsafe conditions such as snagging of the skin.

Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the kitchen, one bottle of carpet cleaner labeled " Keep out of reach of children" was on the floor in a plastic container accessible to the children.
2. In the kitchen, one bottle of multipurpose cleaner labeled "Physical and chemical hazard" and " Keep out of reach of children" was on the floor in a plastic container accessible to the children.
3. In the kitchen, one bottle of antifreeze and coolant labeled "Harmful or fatal if swallowed or inhaled" and "Keep out of reach of children" was on the floor in a plastic container accessible to the children.
4. Children enrolled in the center access the kitchen as a route to the playground.

Description: Based on observation and teacher interview, the facility did not ensure that areas used by children of preschool age and younger shall have protective covers on all electrical outlets.

Evidence:
1. In the hallway access to the playground, the electrical outlets were not covered.
2. The toddlers and two year olds use the hallway to access the playground.

Description: Based on observation and interview, the center did not ensure that cribs shall be identified for use by a specific child.

Evidence:
1. In classroom #2, four out of five cribs were not labeled for use by a specific child.
2. There were four infants in attendance.
3. The labeled crib was for a child that was not attending on 08/16/2019.
