Comments:
An unannounced renewal inspection was conducted today from 12:00pm-2:30pm. There were 122 children directly supervised by 20 staff. The physical plant, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, listening to an audiobook, and preparing for nap time. Hand-washing procedures were also observed. There was a supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous items were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Venus and Earth classrooms, disinfecting wipes were open and placed on a counter accessible to children.

Plan of Correction: Wipes will be moved to a locked cabinet today.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the provider failed to implement a monthly practice evacuation drill.

Evidence: A drill for August 2019 was not documented in the monthly practice evacuation drill log.

Plan of Correction: We will document all drills going forward. A drill was conducted on 8/22/2019/

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center does not have documentation of completion of the required fingerprint criminal history record check.

Evidence:
1) Staff 1 does not have a completed fingerprint based criminal history record check. (Staff 1, DOH 2/19/19)
2) Staff 2 does not have a completed fingerprint based criminal history record check. (Staff 2, DOH 8/19/19)
3) Staff 3 does not have a completed fingerprint based criminal history record check. (Staff 3, DOH 8/12/19)

Plan of Correction: We thought we could utilize FBI finger prints from their previous employers. The staff will be signed up for new fingerprints as soon as possible
