Technical Assistance:
The climbing rock wall on the center's playground was discussed.

Comments:
An unannounced Monitoring Inspection was conducted on September 11, 2019 from approximately 8:55 am until 12:20 pm. Upon entrance there were 23 children and 3 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 2 staff records. Children were observed during snack. Areas of non-compliance are identified on the violation notice. Please contact the licensing inspector with any questions (757) 404-0568.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on documentation review of five children's records and staff interviews, the licensee did not ensure the center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.

Evidence: The following records were reviewed determined to be incomplete;
1) The record for child #1, child #2, child #3 and child #5 did not include the previous childcare/school and did not include schools/programs currently attending.
2) The record for child #1 did not include the home phone number for one parent listed.
3) The record for child #4 did not include the street name, house or apartment number or zip code for one emergency contact listed.
4) The record for child #5 did not include the home addresses for both emergency contacts listed.

Plan of Correction: The center director will have the records updates.

Standard #: 22VAC40-185-510-E
Description: Based on observation and staff interview, the licensee did not ensure medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: One medication, Epi-Pen was observed and without a child's name, the dosage amount and the time or times to be given. Center Director confirmed the medication belonged to child #6, currently enrolled in the program. .

Plan of Correction: The center director will send home the medication.

Standard #: 22VAC40-185-510-J
Description: Based on observation and staff interview, the licensee did not ensure medications, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Five medications were observed and they were stored in an unlocked cabinet draw. Center Director confirmed the draw had been unlocked earlier this morning and they had not had a chance to re-lock it today.

Plan of Correction: The center director will send home the medication.

Standard #: 22VAC40-185-550-C
Description: Based on observation and staff interview, the licensee did not ensure the emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Evidence: The emergency evacuation procedures posted do not include the secondary means of egress.

Plan of Correction: The center director will update the plan..

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. and include all required information.

Evidence: One injury report was available for review and it dd not include the time the parent was notified of the injury.

Plan of Correction: The center director will talk to the staff.
