Comments:
An unannounced monitoring inspection was conducted beginning 8/19/19 and concluding 8/28/19 to investigate a self-reported incident received by the Northern Licensing Office regarding supervision. The investigation was conducted in conjuction with another local agency. Staff and child records were reviewed, observation of the playground and Preschool 2 classroom was completed, staff written statements were obtained, staff interviews were conducted and the center's video recording was reviewed. Violations were found regarding staff supervision related to the self-reported incident as well as an unrelated violation. Please provide a plan of correction for the noncompliance and include the date to be corrected. Please return a signed and dated copy to me by Thursday, September 12, 2019 at 5pm. Standard #: 22VAC40-185-40-E
Description: Based on staff interviews, documentation review and observation of the center's video recording, staff did not follow the center's own policies and procedures as required.

Evidence:
The center's Name to Face policy requires staff to "actively match a child's name on the daily attendance sheet to their face in the classroom. During transitions, the teacher is required to complete a transition portion of the Attendance Roster." The director stated that during each transition, there are 4 boxes on the Name to Face Roster that are to be checked and include: Name to Face as the children are leaving the classroom, in the hallway outside of the classroom, lined up once they've reached their destination, and finally once they enter the destination room or area.
1. On 8/13/19, Staff #1 and Staff #2 did not complete the required Name to Face documentation and protocol on two separate occasions as the children transitioned from the playground back to the Preschool 2 Classroom which resulted in a child being left behind on the playground each time.
2. The Preschool 2 class returned to the building from the playground at approximately 9:37am on 8/13/19. The Name to Face procedure and documentation was not completed during the transition from the playground back to the classroom. Staff #3 was in an adjoining fenced in area with a different class when she noticed Child #1 (Age 2) was "standing by the fence looking at the big playground crying". Staff #3 knocked on the Preschool 2 classroom window to get the teacher's attention. Staff #4, who had stepped in for Staff #1, came to the playground at 9:39am and took Child #1 back to the Preschool 2 classroom.
3. The Preschool 2 classroom went out for a second time on the playground and returned to the building at 11:05am. Staff #1 and Staff #2 did not complete the Face to Name Procedure and Child #2 (Age 2) was alone on the playground for approximately 18 minutes until Staff #2 looked out the window of the classroom and saw the child outside.

Plan of Correction: We plan to train all teachers again on the Name to Face Policy and on Proper Hand Washing Procedures; as well as when to wash hands. Managers will be monitoring and signing off on Name to Face Sheet 3 times a day. Management will also be observing the Name to Face being performed every time the teachers and children transition from classroom to playground/ playground to classroom. We plan to continue to observe the transitions until we have continuously observed progress with the teachers completing the Name to Face policy and the assurance that it is being followed correctly daily. Every month thereafter, we will randomly pull the classroom Name to Face Sheets to ensure that the Name to Face Sheets is being completed properly throughout the day and evening.

Standard #: 22VAC40-185-180-B
Description: Based on staff interviews, review of staff written statements, and observation of the center's video recording, the center did not ensure that staff shall be of good character and reputation and capable of communicating effectively both orally and in writing as applicable to the job responsibility.

Evidence:
1. Staff #1 and Staff #2 were not truthful in their accounts of how long Child #2 was alone on the playground and the procedures followed based on the video recording viewed.
2. Staff #1 stated "At 11:05 I lined my kids up outside. While sitting them for name to face, a few of my students ran into the classroom. I told Staff #2 to check the playground to gather up the rest of the children while I went into the classroom with the kids that ran off." Video recording does not show the children lining up or sitting down. Staff #1 opened the door to the building as children began to gather around and 1 child walks in, followed by Staff #1. The remainder of the children, excluding Child #2, follow Staff #1 inside with Staff #2 being the last to enter the building.
3. Staff #1 stated "I walked over to the counter and did Name to Face. I started calling names and realized he (Child #2) wasn't there. I dropped everything and ran to get him". Child #2 was not found alone on the playground as a result of the Name to Face procedure being completed, instead, Staff #2 happened to look out of the window and saw the child after approximately 18 minutes.
4. Staff #1 stated Child #2 was outside alone for about 4 minutes. Staff #2 stated "it was instant, it was maybe a minute" once she saw Child #2 outside alone.
5. The Preschool 2 Name to Face Attendance Roster was reviewed and had all required check marks for each transition from the classroom to the playground, suggesting Name to Face was done. Staff #5 stated she checked the Preschool 2 Classroom's Name to Face Roster at approximately 10:30am and no checks were on the form, however, later in the day on 8/13/19 checks were found on the Name to Face Roster.

Plan of Correction: We plan to train all teachers again on the Name to Face Policy and on Proper Hand Washing Procedures; as well as when to wash hands. Managers will be monitoring and signing off on Name to Face Sheet 3 times a day. Management will also be observing the Name to Face being performed every time the teachers and children transition from classroom to playground/ playground to classroom. Description: Based on staff interviews, staff written statements and observation of the center's video recording, staff did not ensure the care, protection and guidance of children in their care.

Evidence:
1. At approximately 9:37am, after the Preschool 2 class, along with Staff #1 and Staff #2, entered the Preschool 2 Classroom from the playground, Staff #4 came to step in for Staff #1 while the staff went for a bathroom break. Staff #1, upon returning from the bathroom break, was not informed by either Staff #2 or Staff #4 of the incident when Child #1 was left on the playground.
2. While Child #2 was outside alone on the playground for approximately 18 minutes, rain is visible on the center's video recording system making the concrete wet. When Child #2 returned to the classroom, Staff #1 was observed via video recording drying the child?s head off.
3. Staff #1 and Staff #2 were not aware Child #1 (Age 2) was left behind on the playground at approximately 9:37am until Staff #2 finally looked at the Attendance Roster after multiple counts of the children were not correct. Staff #1 and Staff #2 were not aware Child #2 (Age 2) was left behind on the playground at approximately 11:05am for 18 minutes until Staff #2 looked out the window to the playground and saw Child #2 alone outside.

Plan of Correction: We plan to train all teachers again on the Name to Face Policy and on Proper Hand Washing Procedures; as well as when to wash hands. Managers will be monitoring and signing off on Name to Face Sheet 3 times a day. Management will also be observing the Name to Face being performed every time the teachers and children transition from classroom to playground/ playground to classroom. Description: Based on staff interviews, review of staff written statements, record review, and observation of the center?s video recording, one child was not always within actual sight and sound supervision of staff.

Evidence:
1. On 8/13/19, Child #2 (Age 2) was left alone on the playground for approximately 18 minutes after his class transitioned back to the Preschool 2 classroom.

Plan of Correction: We plan to train all teachers again on the Name to Face Policy and on Proper Hand Washing Procedures; as well as when to wash hands. Managers will be monitoring and signing off on Name to Face Sheet 3 times a day. Management will also be observing the Name to Face being performed every time the teachers and children transition from classroom to playground/ playground to classroom. We plan to continue to observe the transitions until we have continuously observed progress with the teachers completing the Name to Face policy and the assurance that it is being followed correctly daily. Description: Based on review of the center's video recording, children's hands were not washed with soap and running water or disposable wipes before eating meals.

Evidence:
1. Upon returning to the classroom from the playground on 8/13/19, the Preschool 2 class began eating lunch at approximately 11:13am. The children in the class did not wash hands before beginning to eat.
2. Child #2 was returned to the classroom at approximately 11:23am and was immediately seated for lunch. Child #2 also did not wash hands before beginning to eat lunch.

Plan of Correction: We plan to train all teachers again on the Name to Face Policy and on Proper Hand Washing Procedures; as well as when to wash hands. Managers will be monitoring and signing off on Name to Face Sheet 3 times a day. Management will also be observing the Name to Face being performed every time the teachers and children transition from classroom to playground/ playground to classroom. We plan to continue to observe the transitions until we have continuously observed progress with the teachers completing the Name to Face policy and the assurance that it is being followed correctly daily.
