Comments:
An unannounced monitoring inspection was conducted on 9/12/19 from 9:25am - 11:45am. During the inspection there were 59 children ages two years old through five years old in care with ten staff. Children were observed participating in various activities in the classrooms, and playing outside. Records were reviewed for six children and six staff. Medication, emergency procedures, and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of six children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #6 did not contain documentation for viewing the proof of the child's identity and age. 2. Staff #7 (Program Director) confirmed that the record for child #6 did not contain all of the required information.

Plan of Correction: The facility responded: The parents of child #6 have been contacted and asked to provide a copy of his birth certificate. children's records will be reviewed to ensure all are complete.

Standard #: 22VAC40-185-510-N
Description: Based on a review of the medication being stored at the center, it was determined that the licensee did not ensure that when an authorization expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: 1. The medication authorization for child #7 expired on 1/31/19, but the medication (Benadryl) was still at the center. 2. Staff #1 confirmed that the medication for child #7 had been present in the center for more than 14 days since the expiration of the written authorization.

Plan of Correction: The facility responded: The parent was contacted to pick up the medication. All medication will be sent home within 14 days of expiration oft he medication authorization.

Standard #: 22VAC40-185-550-M
Description: Based on a review of 20 injury reports and interviews, it was determined that the facility did not ensure that the written record of children's serious and minor injuries includes all of the required items. Evidence: 1. Five of the twenty injury reports reviewed did not include the action to prevent recurrence of the injury. 2. Staff #7 (Program Director) reviewed the injury reports and confirmed there were five injury reports that did not include all of the required information.

Plan of Correction: The facility responded: Staff will be re-trained on how to complete injury reports. center management will review all injury reports to ensure they are complete.
