Comments:
The inspector conducted an unannounced focused inspection on 09/10/2019 from 12:23pm to 3:15pm. This inspection focused on compliance with children's records, staff records, qualifications and trainings. Interviews with staff were completed, records were reviewed and observations were made at the center. There were two violations resulting from this inspection.

Five children?s records and two staff records were reviewed during this inspection.

Today, the following child to staff ratios were observed:

Infants / Toddlers - 5:1
Preschool (Twos, Threes and Fours) - 5:1

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not ensure two of two staff records contained all of the required documentation.
Evidence:
1. The record of staff #1 (start date: 4/30/19) did not have documentation of a high school program completion or equivalent as required by the job position. The center's representative informed the inspector that staff #1 was a program leader. The inspector observed staff #1 supervising 5 children alone.
2. The record of staff #2 (start date: 8/26/19) did not have documentation showing that the staff has completed the required orientation training.

Plan of Correction: 1. We could not locate the copy and will have a new copy by tomorrow.
2. Will complete the orientation checklist by Thursday.

Standard #: 22VAC40-185-350-C
Description: Based on observation, the center did not ensure when children are regularly in ongoing mixed age groups, the staff to children ratio applicable to the youngest child in the group shall apply to the entire group.
Evidence:
The inspector observed staff #1 supervising five infant and toddler children from 12:23pm to 1:36pm. The center's representative informed the inspector that the infant and toddler children are normally combined during nap time.

Plan of Correction: We will always keep a teacher and a teacher assistant in the room with five children or more.
