Technical Assistance:
Renewal Application and fee have not been received as of the inspection date. The license renewal application is on-line at the VA DSS website if needed.

Comments:
An unannounced renewal study was conducted today. This was the first day of the program operation for the 2019-2020 school year. Thirteen children were present with two staff and was within ratios for supervision. Observed children finishing lunch/snack, hand washing, puzzles, choosing books to take home and coloring. Room is well equipped with materials and equipment for preschool children. Center staff have first aid/CPR and Medication Administration Training (MAT) certification. Description: One of three children's records is incomplete.
Evidence:
Child #1 is missing a second emergency contact address.

Plan of Correction: Center Director will reach out to the parent at pick-up.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the room are not maintained is a clean, safe or operable condition.

Three ceiling tiles are water stained and a small patch of paint is peeling/popped below the window.

Plan of Correction: Maintenance request has already been requested from County Facilities.

Standard #: 22VAC40-185-510-A
Description: Based on record review, an over the counter (OTC) medication did not have written authorization from the parent.
Evidence:
Child #2 had an OTC medication on-site but the parent had not completed the authorization.

Plan of Correction: The center director will request the parent complete the authorization at pick-up today.
