Technical Assistance:
Discussed ways to capture annual review of children records.

Comments:
An unannounced renewal inspection was conducted today from 9:00 am-1:00pm. There were 108 children (infants-5yrs.) supervised by 21 direct care staff. Children were observed participating in story time, circle time, puzzles, legos, outside playgroup play, and eating breakfast. Diapering and hand-washing procedures were also observed. A selection of staff and children records, medications and other documents were reviewed. The playground and surfacing was inspected. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on staff records review, the center has not maintained Tuberculosis Testing (TB) at least every two years. Evidence: Staff #1 TB screening is dated 7/27/16.

Plan of Correction: Form was given to staff, staff made an appointment.

Standard #: 22VAC40-185-420-E-3
Description: Based on children's record, the center did not maintained annually parent confirmation that the required information in the child's record is up to date. Evidence: Child #1, #2, #3, #4, #5, #6, and #7 did not have annual updates of required information.

Plan of Correction: We will send add signature and date for parent to update required information.

Standard #: 22VAC40-185-520-A
Description: Based on observation, over-the-counter skin products were kept or used beyond the expiration date of the product. Evidence: Child #6 and #9 both had diaper creams with an expiration date of 8/2019.

Plan of Correction: We will send parent authorization with children today.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center has not maintained written parent authorization for all diaper ointments and creams. Evidence: There was no written parent authorization form for Child #9's diaper cream.

Plan of Correction: We will send expired medication home today . I will request parent to bring new form if want continue.
