Comments:
An unannounced monitoring visit and facility tour was conducted from 10:50 am- 12:20 pm. At the time of inspectors' arrival there were 28 toddler to school aged children in care with 6 staff members and 1 administrator. Children were observed participating in indoor play, educational activites, transitioning between activities, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview it was determined the center did not ensure the record for each child contained all of the required components. Evidence: 1. The record for child #3 did not contain all of the information for one of the two emergency required emergency contacts. 2. There was no documentation of annual record review by parents for child #2, #3, #4, and #5 . 3. The director confirmed this information was not in the record for child #3

Plan of Correction: Director will request the information from the parent at the time of pick up and have all parents complete annual registration paperwork.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure all ares and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the outdoor play area there was: - two tricycles with broken bells on the handlebars - three tricycles with broken red plastic seats - an empty gingerale can that was crushed and had sharp edges - the door to the outdoor space had peeling paint on the exterior side 2. Nine 3-4 year olds were observed playing in this area during this inspection.

Plan of Correction: Broken items will be disposed of or repaired. Door will be repainted.

Description: Based on record review it was determined the center did not ensure that practice drills were recorded for the year. Evidence: 1. The bi-annual shelter in place drills were not documented. 2. The director confirmed that the drills were not documented.

Plan of Correction: Director will ensure that all drills are documented.
