An unannounced monitoring inspection was conducted with center director and staff to determine compliance with licensing standards. There were 24 preschool children with two staff and 60 school age children present with five staff supervising. The preschool children were playing with age appropriate toys and listening to a story. The school age children were arriving from school, having afternoon snack, and working on homework. Snack served today: crackers and orange slices. Four staff files and eight children's files were reviewed - please see Supplemental Summary for details. Three emergency medications for three children were reviewed and are in compliance. The last health inspection was conducted 2/26/2019. The last fire inspection was conducted 5/10/2019. Evacuation drills have been conducted monthly; the last shelter in place was conducted 7/11/2019. Standard #: 22VAC40-185-60-A Description: Based on review of eight children's files, the facility failed to have all information in each child's file. Evidence: Child 4 did not have the address of one emergency contact and Child 8 did not have the phone number or address of two emergency contacts to call in an emergency if a parent cannot be reached. Plan of Correction: Center director will request the information from the parents.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation, the facility failed to maintain the required ratio of one staff member for every 10 children ages three years to the age of eligibility to attend public school (five years by September 30). Evidence: between 2:47 and 3:05 p.m., there were 24 children, (ages three and four) present with two staff. Plan of Correction: The center is short staffed today and one teacher had to make a bus run to pick up school age children.
