Technical Assistance:
The diapering container used to dispose of soiled diapers must be next to the diapering surface and accessible so the staff can easily step on the foot pedal leaver to open.

Tummy time for those infants who can flip over on their own should be 30 minutes a day.
The attendance log must be up-to-date and accurate at all times.

Comments:
An unannounced monitoring inspection was conducted today with the directors from 10:50 am until 1:20 pm.Fifty five children were present with ten staff providing supervision.Two preschool classrooms were outside enjoying the sunny day while other children were inside preparing for lunch to be served. The Licensing Inspector was able to observe lunch being served, which consisted of turkey or ham cheese melt, pears, corn and milk.
Six children and staff files were reviewed as well as nine medication for five children. A sample size of 6 injury/accident reports were reviewed as well.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, it was determined that hazardous substances were not kept locked.
Evidence:
The Licensing Inspector observed two containers of sanitizing solution and disinfectant wipes unlocked in a cabinet in the 3/4 year old classroom.

Plan of Correction: We will retrain all staff on the importance of keeping hazardous substances locked.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that there was not at least six inches of mulch underneath and surrounding outside play equipment..
Evidence:
The Licensing Inspector observed compacted mulch and mulch that had turned to soil underneath and surrounding the two merry-go-rounds on the 2/3 year old playground, consequently the mulch was no longer considered a resilient surface.

Plan of Correction: We will place the merry go rounds off limits until we can either rake or add additional mulch to ensure at least 6 inches underneath and surrounding the play equipment.
