Comments:
An unannounced monitoring inspection was conducted on 09/16/2019 from 11:35 am - 1:45 pm. At the time of entrance there were 90 preschool children in care with 11 staff. Children were observed during lunch nap. Hand washing and toileting procedures were observed after completion of lunch. Records were reviewed for nine children. Medication and emergency supplies reviewed.
Violations were observed in four parts of the CDC standards and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that outside areas and equipment of the center shall be maintained in a safe condition.

Evidence:
1. The following safety hazards were observed on the outdoor play area:
a. There were multiple areas of peeling paint located on the metal support bars inside the upper interior ceiling of the playhouse. These areas are within reach of the preschool children in care and are hazardous if ingested. Additionally, the paint has areas of sharp rust which could potentially cut children's fingers.
b. The wooden board, located in the grassy area adjacent to the back fence, has widespread areas of peeling and broken wood. This is a trip hazard.

Plan of Correction: The playhouse will be repaired and painted by 10/15/19.
The wooden board located in the grassy area will be replaced by 10/15/19.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and interview, the center failed to maintain the required staff-to-child ratio for children from three years to the age of eligibility to attend public school, five years by September 30: one staff member for every 10 children.

Evidence:
1. Staff 2 was alone with a group of 13 preschool children when classroom pink-A was viewed at 11:40 am.
a. Staff 2 stated that a custodial staff person had been sent to help her however, this individual was observed outside on the front deck of the facility when the inspector arrived at 11:35 am.
2. Staff 2 stated she was not sure where the custodial staff person was when the inspector asked.
3. The custodial staff came into the classroom at 11:45 am.

Plan of Correction: A permanent second staff has been hired for classroom A starting 9/18/19. A retraining on staff-to-child ratio will take place on 9/17/19.

Standard #: 22VAC40-185-430-C
Description: Based on observation, the center failed to ensure that play equipment used by children shall have no pinch points.

Evidence:
1. The two metal steering wheels on the outdoor play car have loose bolts. Both steering wheels no longer fit flush to the equipment which is a potential pinch point for children's fingers.

Plan of Correction: Both metal steering wheels will be repaired to fit flush.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables shall be sanitized after meals. *Sanitized means that the surface of the item is sprayed with the disinfectant solution and allowed to air dry.

Evidence:
1. The tables in classroom F were sanitized after completion of lunch however, they were not allowed to air dry as required.
a. After spraying each table with a disinfectant solution a paper towel was used to wipe the tables.

Plan of Correction: Retraining will take place to ensure that proper sanitizing takes place in the future.
