Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, August 20, 2019 from approximately 1:00 PM until 3:25 PM. The census for today's inspection consisted of 29 children. During the inspection, children were observed having tummy time, napping, eating snack, reading stories and coloring. Children's equipment, learning materials, outdoor and indoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Five children's records and five staff records, annual fire and health inspections, liability insurance policy, fire drill and shelter in place drill logs, daily attendance, staff training and staff development activities and all required postings were reviewed.

The assistant director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children's records, the provider did not ensure each record contained documentation of immunizations prior to attending the child day center.

Evidence:
Child #3 began attending the center on 05/06/19. Documentation of immunizations was received 06/10/19.

Plan of Correction: "Make sure all children's files are completed before they start daycare."

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
1. The record for staff #2 (date of last TB screening 05/15/17) did not contain updated TB screening results as of 08/20/19.
2. The record for staff #3 (date of last TB screening 05/15/17) did not contain updated TB screening results as of 08/20/19.
3. The record for staff #4 (date of last TB screening 07/31/17) did not contain updated TB screening results as of 08/20/19.

Plan of Correction: "Called Health Department today and scheduled T.B. Health screenings for the three employees for August 30, 2019."

Standard #: 22VAC40-185-450-A
Description: Based on observation, the center did not ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
In classroom #2, classroom #3 and classroom #4 children did not have a bottom cover during the designated rest time.

Plan of Correction: "Ask parents to bring children two blankets from home for rest mats."
