Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 1:15 PM and departed at 3:45 PM. Twenty-two children were in care. Activities and nap time were observed. A sample size of six staff records and five children's records were reviewed. Medication administration and injury records were reviewed.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based upon review of six staff records and staff interview, the facility has not ensured that the record for each staff includes written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position.
Evidence:
1. The record provided for staff 6 does not include documentation verifying the programmatic experience and training of staff 6, a program leader.
2. Staff 1 verified that the required documentation was not in the record of staff 6.

Plan of Correction: The facility responded with the following:
All staff records will include complete documentation of education and experience required of the position.

Standard #: 22VAC40-185-280-B
Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
1. The hand soap on the sink of the unlocked staff restroom was labeled "caution, keep out of reach of children".
2. Under the sink in the unlocked staff restroom there was a gallon container of Healthy Hands hand soap Labeled "caution, keep out of reach of children".
3. In an unlocked upper wall hung cabinet in the middle storage area there were two containers of aerosol appliance epoxy. The cans were labeled "warning, vapor harmful, keep out of reach of children".
4. Staff 7 acknowledged that the above hazardous substances were not in locked locations.

Plan of Correction: The facility responded with the following:
Hazardous substances were moved to locked locations. All staff will be reminded of this expectation. Routine checks will be conducted to ensure that all hazardous substances are in locked location.

Standard #: 22VAC40-185-330-B
Description: Based upon observation and measurements, the facility has not ensured that where playground equipment is provided, resilient surfacing complies with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 requiring sufficient resilient surfacing under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence:
1. The facility has added a series five of playground stepping pods. The resilient surfacing (wood mulch) under the stepping stones was measured in four separate areas to be 1 inch in depth before turning into decayed mulch/soil. A minimum of six inches of wood mulch is required in the fall zones of the pods.
2. The facility has a tall climbing structure for use by school aged children. The resilient surfacing (wood mulch) under the structure was measured in five separate areas to be 1 to 2 inches in depth before turning into decayed mulch/soil. A minimum of
six inches of wood mulch is required in the fall zone of the climber.

Plan of Correction: The facility responded with the following:
Resilient surfacing will be ordered and spread to the required depth in the fall zones of all climbing equipment.
