Comments:
An unannounced Monitoring Inspection was conducted today from 10:20am to 12:20pm. There were 53 (infants to school-agers) directly supervised by 11 staff. The physical plant, 4 staff records, 7 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-440-E
Description: Based on observation, the facility did not ensure that occupied cots have at least 12 inches of space between them. Evidence: Observed in the 12-18 month classroom, two occupied cots that were less than 12 inches apart.

Plan of Correction: One of eight cots were not 12 inches apart. We have created a new cot arrangement to allow for cots to be 12 inches apart.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the facility did not ensure that diaper ointment was in the original container and labeled with children's names. Evidence: Observed 3 diaper creams that were not labeled with children's names in Older 2s and Infants classroom.

Plan of Correction: Staff will assist parents in labeling ointment prior to leaving school.
