Thank you for your assistance during this announced monitoring inspection. This on-site visit was conducted on September 19, 2019 from 2:50 p.m. through 4:30 p.m. There were thirty-eight children in the care of five staff. The medication and medication paperwork was reviewed for one child. Five children's and seven staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice.
Areas observed were: arrival, outside play, hand washing/bathroom procedures, snack, table sanitizing, and homework. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to document the first date of attendance for five children.
Evidence: 1. Five children's records were reviewed. 2. There was no documentation of the first date of attendance for Child A, B, C, and E. 3. The director confirmed the first date of attendance had not been documented.Plan of Correction: All children's records will contain documentation of the first date of attendance.
