Comments:
An unannounced inspection was conducted to investigate a complaint related to linens, supervision, and care, protection, and guidance of the children in care at the center. The investigation was initiated on July 30, 2019 with the Assistant Director beginning at 12:50 PM and ending at 2:00 PM. Staff interviews were also conducted on July 31, August 2, and August 14, 2019. An exit meeting was conducted with the District Director on September 24, 2019. Documentation and staff records were reviewed. Classroom observations were conducted. It was determined that the complaints regarding linens, supervision, protection, and guidance were valid.

Standard #: 22VAC40-185-40-E
Complaint related: Yes
Description: Based on staff interviews and documentation review, staff failed to follow the center's own policies and procedures as required by by the standards. Evidence: The center's Face to Name policy for accounting for children at all times requires staff to compare the children's faces to the name listed on the written attendance during transitions from different areas of the facility. Staff A did not complete a name to face check of all the children after transitioning from the lobby/hallway to the outside play area. Child A was left unsupervised in the lobby/hallway area for approximately a minute as the class exited the building and entered the play area.

Plan of Correction: Although child was out of sight and sound of Staff A, the child was never unsupervised as the District Director was within a few feet in the lobby.

Complaint related: No
Description: Based on a review of documentation, it was determined that not all staff records were complete. Evidence: Staff A's record was missing documentation of two reference checks prior to employment.

Plan of Correction: Staff A is no longer with the company based on the incidents of July 17th and 30th. All current staff have 2 reference checks on file.

Complaint related: Yes
Description: Based on staff interviews, staff members failed to ensure the care, protection, and guidance of children in care. Evidence On July 17, 2019 staff A and B left thirteen two year old children alone in a classroom during nap time for approximately 5-10 minutes. The staff were unaware that the class was left unsupervised as staff A had not communicated to staff B that she was leaving the classroom. Staff B left the classroom without confirming that staff A was still in the classroom.

Plan of Correction: Staff B entered room to check on Staff A. Staff B was not scheduled to be in the room at nap time. Staff B took a call in the room and had her back to the classroom. At that moment, Staff A left the room to receive a child from the county school bus that had arrived in the parking lot. Staff B ran out assuming Staff A would stay in the room. Staff B was unaware Staff A had left, because she left out the door that led to parking lot and not the classroom door. During this miscommunication between Staff A and B, all children were sleeping on their cots. When Staff A returned moment later, all the children were still sleeping. Two weeks later, when upper management was made aware of this incident, staff A and B were immediately placed on administrative leave. This incident resulted in termination of employment.

Complaint related: Yes
Description: Based on staff interviews, staff failed to ensure that all children under the age of 10 years old were within actual sight and sound supervision of staff. Evidence: On July 17, 2019, staff A and B left thirteen two year old children alone in a classroom during nap time for an undetermined amount of time ranging between 5-10 minutes. On July 29, 2019 staff A left a two year old child unsupervised in the hallway for approximately one minute as the classroom exited the building to the outside play area.

Plan of Correction: As previously stated, both staff members are no longer with the company. All current staff members were retrained on sight and sound and Name to Face to ensure all children under 10 are within sight and sound of staff members.

Complaint related: Yes
Description: Based on observation, not all of the cots used by children during rest periods had linens that consisted of a top and bottom cover or a one-piece covering which is open on three edges. Evidence: On July 30, 2019 the licensing inspector observed that there were two children on cots without bottom linens in the two's classroom and seven children on cots without bottom linens in the prepper's room.

Plan of Correction: Center has purchased extra linens to have on hand when parents forget to bring them.
