Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:00 AM and departed at 2:25 PM. Eighty-three children were in care with ten staff. Educational activities, lunch, and nap were observed. A sample size of eight staff records and eight children's records were reviewed. Medication administration was reviewed. Areas of noncompliance are detailed in the violation notice and were discussed with staff #8 at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview it was determined the center did not ensure the record for each child enrolled contained all of the required information. Evidence: 1. The record for child #4 did not contain one of the two required emergency contacts. 2. Staff #8 confirmed child #4's record only contained one of the two required emergency contacts.

Plan of Correction: Staff will get the required emergency contact information from the parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was broken floor tiles in the hallway outside the four year old classrooms and by the door to the outdoor play area. 2. There was peeling paint on the wall in staff #8's classroom and in the rest room inside staff #1's classroom. 3. There was peeling drywall in the rest room of staff #4's.

Plan of Correction: Staff will submit a maintenance request for the needed repairs.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: There were unlocked chemicals accessible to the children in care in staff #7's and staff #4's classrooms.

Plan of Correction: Staff will be reminded to make sure all chemicals are kept in a locked area.

Standard #: 63.2(17)-1721.1-B-2
Description: Based upon review of five staff records, the facility has not ensured that all employees have undergone a background check in accordance with 63.2-1716 requiring nationwide fingerprinting. Evidence: The record provided for staff #8, on duty with the children during the inspection, did not include the results of fingerprinting.

Plan of Correction: Staff #8 was told the previously submitted fingerprints were returned. Staff #8 did not receive the returned fingerprints that were submitted in April 2018. Staff #8 will schedule to resubmit fingerprints.
