Comments:
An unannounced monitoring inspection took place today, September 27, 2019, beginning at 2:45 PM and ending at 5:00 PM. There were 22 children in care today with three staff providing care and supervision. There were no medications onsite during the inspection. The playground area and all rooms that are used for care were inspected, as were five children's records and all three staff records. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children were observed during arrival, homework time, snack and craft time. Staff were observed during activities, during other engagement with the children and cleaning and sanitizing practices. There were discussions about childre'?s records and the menu. Violations were discussed at the conclusion of this inspection. Description: Based on record review and discussion with staff, the center failed to ensure that the center shall maintain and keep at the center a separate record for each child enrolled which shall contain all elements as required by the standards.

Evidence:

1. Children (1, 2, 3, 4 and 5) did not have addresses for custodial parent(s) or guardian(s). The form did not have an option available for a parent or guardian to list an address that may differ from the child's address.

Plan of Correction: Staff plans to write in the appropriate information on the form.
