Licensing inspector arrived at 4:45pm and departed at 5:50pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. Upon arrival there were 27 children in care with 3 staff . Children were observed participating in various table activities, cleaning up and leaving to go to home. The sample size consisted of 4 children records .
Standard #: 22VAC40-185-130-A Description: Based on record review, the licensee did not ensure a immunization record was obtained before a child attends the center .
Child #1 and child #3 attended the center on September 25, 2019 without documentation of immunization record.
Plan of Correction: The site director will ensure all shot record are obtained prior to the first day of care
Standard #: 22VAC40-185-60-A Description: Based on record review of five children's records and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which shall contain all the information.
The record for Child #1 did not include the physicians name and phone number, previous child day care and schools attended by the child;and name of any additional programs or schools that the child is concurrently attending .
The record for Child #3, did not include both emergency contact address, father's name, phone number, address, father's employers name and phone number,previous child day care and schools attended by the child;and name of any additional programs or schools that the child is concurrently attending and allergies.
Plan of Correction: The cite director will continue to follow up with parents to ensure all information for children's files is provided and documented in files
Standard #: 22VAC40-185-90--A Description: Based on record review and staff interview, the licensee did not ensure the written agreements between the parent and the center were in each child's record by the first day.
Child #1 did not have written agreements between the parent and the center in the record. Staff stated the record was missing items.
Plan of Correction: The site director will ensure all written agreements new received prior to the first day of care.
Standard #: 22VAC40-185-540-E Description: Based on observation and staff interview, the licensee did not ensure there was a working battery operated radio and working battery operated flashlight in the center.
Evidence: Staff #1 stated the center did not have a battery operated radio and flashlight. There was order made to the main office to have flashlight and radio purchased a week ago.
Plan of Correction: Flashlights and radio were received on 9/27/2019 and are being stored on site.
Standard #: 22VAC40-185-550-D Description: Based on record review, the licensee did not ensure the center monthly practice drill was completed.
Evidence: The center did not have documentation that a monthly dril was completed for April, May and June.
Plan of Correction: The site director will ensure that monthly fire drills are conducted.
