An unannounced renewal inspection was completed on September 6, 2019 from approximately 12:15 p.m. to 3:15 p.m. Twenty-one children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, reading, singing, preparing for nap time, departure from the center, and nap time. Interviews and interactions were conducted and no medications are given. Five children?s records, five staff records and three corporate officer records were reviewed. The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-140-A Description: Based on a review of children?s records, the licensee failed to obtain a physical for one of five children before the child?s attendance or within one month after attendance.
Evidence: 1. Child #2's record, enrolled on January 9, 2019, lacked documentation of a physical. 2. Administrator #1 acknowledged a physical was not in the record.
Plan of Correction: The director will contact the parent today to obtain a copy. Director reports that the child's siblings, who started the same day, have physicals in their records so she knows it has been done and it was an oversight that it was not obtained from parent.
Standard #: 22VAC40-185-550-M Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence: 1. Injury reports since August 7, 2019 were reviewed. Of the twenty reports reviewed, approximately six lacked all the required information. 2. Administrator #1 acknowledged these reports were missing required information.
Plan of Correction: The director will remind staff today about all the requirements for injury reports.
