Comments:
An unannounced Monitoring Inspection was conducted on October 1, 2019 from approximately 4:50 pm until 6:00 pm. Upon entrance there were 10 children and 2 staff present with children. The record review sample consisted of 5 children's records and the emergency information, health related information and training information for 2 staff records. Four medications were on site and reviewed. The Center Director confirmed they are not providing transportation. Children were observed playing basketball in the gym and cards games in the multipurpose room. The area of non-compliance is noted on the violation notice. Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence: There is not documentation of an annual inspection available for review, the most recent fire inspection available for review is dated 5-31-2018.

Plan of Correction: The director will get a copy of the fire inspection by tomorrow.
