Technical Assistance:
Standard 22 VAC 40-185-60A (children's records), as well as background check regulations, were discussed during today's inspection.

Comments:
An unannounced monitoring inspection was conducted on 10/4/19 from 10:00am until 12:30pm. At the time of the tour, there were 42 children in care with 7 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in morning learning activities and eating lunch. Restroom, hand washing and lunch service procedures were also observed. Medication, children's injury reports, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and center postings were reviewed. Two violations in the areas of administration and background checks are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The record for staff 2 does not contain the name, address and telephone number of a person to be notified in an emergency.
2. The center director confirmed that the record for staff 2 is lacking emergency contact information.

Plan of Correction: Staff 2 will fill out the emergency contact information sheet upon her return to work on Monday.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center did not ensure that they obtain the results of the fingerprint based background check prior to the first day of employment.

Evidence:
1. Staff 1 has a hire date of 6/20/19. The results of the fingerprint based background check for staff 1 was not received until 6/24/19.
2. The center director confirmed that staff 1 began employment prior to the results of the fingerprint background checks being received.

Plan of Correction: Going forward, all fingerprint based background checks will be obtained before new staff begin employment.
