Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, September 24, 2019 between 9:50 am to 11:47 am to determine the center's compliance with licensing standards. During this inspection, the center's director and owner were present and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in various classroom settings and playground area. The census for today's inspection consisted of children with 4 staff present including the owner. During this inspection, the children in care were observed by the licensing inspector completing a variety of activities to include participating in circle time,engaged in outdoor play, transitioning outdoors, completing handwashing, and actively playing with peers and age appropriate equipment.The center's bathrooms, kitchen, designated classrooms, indoor areas, playground areas, and equipment were inspected by the licensing inspector. The required posting were reviewed by the inspector. The following information was reviewed: daily attendance, emergency supplies, evacuation and shelter-in-place drills, liability insurance policy, and allergy list. There were five children?s records and four staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children's record and interview with staff, the center did not ensure to maintain a record containing all of the following information.

Evidence: In Child #2's record (start date 9/16/19) one of the designated emergency contact documented address was incomplete. Interview with staff confirmed that the information documented in the record was all available to review at this time.

Plan of Correction: Director will ensure that all places are filled out on application is completed even if the address in one slot is the same as the other.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Pre-K classroom, there was a storage cabinet observed unlocked containing disinfecting wipes on a shelf with the labels "keep out of reach of children" and "warning".
2. Exhibit#1 photograph of the disinfecting wipes inside the storage cabinet without safe locking method.

Plan of Correction: All hazardous products will be placed in locked janitorial closet unless in a safe area that has a lock and key.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation, the center did not ensure to follow the staff to children ratio requirements for children from three years to the age of eligibility to attend public school.

Evidence: In the center's Pre-K classroom, there was one staff present with eleven children in care with the youngest child in the group being three years old.

Plan of Correction: Creative Kids will make sure there is a substitute on standby in the event of a call-out.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview with staff, the center did not ensure to maintain a nonabsorbent surface for diapering or changing and the usage of the diapering surface is only for diapering or cleaning children.

Evidence:
1. In the center's infant room, the diapering surface was torn creating a absorbent surface while diapering. Interview with staff confirmed the diapering surface is being used for children in care.
2. In the center's infant room, the diapering surface was observed with a plastic toy placed on the top. Interview with staff confirmed the diapering surface is being used for children in care.
3. Exhibit #1 photograph shows the diapering surface torn during the inspection.
4. Exhibit#2 photograph shows the plastic toy on the diapering surface during the inspection.

Plan of Correction: 1. Diapering changing mat will be replaced with a new one.
2. Supervising staff will remind staff of policy of toys not being allowed on diapering area.
