22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced monitoring inspection from 9:45 am - 12:30 pm. There were thirty three children in care: two infants with two staff, eight toddlers with two staff, eleven two and three year olds with two staff, and twelve three and four year olds with two staff. The director was also present. The children were observed coloring, during morning snacks, playing with blocks, during lunch and preparing for nap time. Seven staff's records were reviewed and five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Description: Based on record review, the center failed to obtain all required information for the children's records.
1. The inspector reviewed five children's records. One of five records review (child record #5) did not contain proof of identity and age. Child's date of enrollment was 09/09/2019.

Plan of Correction: The director will remind the child's parents that documentation of proof of identity and age is needed for the child's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a safe and operable condition.
1. The inspector observed a shelf in the two and three's classroom that had a piece of wood coming loose on the top with a nail exposed. The inspector observed a broken board on the platform of the wood structure on the playground. The board has an opening large enough for a child's foot to fit into. The inspector observed a table with a loose top and an inoperable toilet in the school-age building.

Plan of Correction: The shelf will be removed from the classroom today. The broken board on the wood structure on the playground will be replaced. The children will not use this structure until the board is replaced. The table top will be tightened and the toilet will be repaired in the school-age building.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review, the center failed to practice an evacuation drill for the month of September 2019.
1. The inspector reviewed the drill documentation. There was no drill documented for September 2019.

Plan of Correction: The center will practice two drills in October to make up for the missing drill.

Standard #: 22VAC40-185-570-G
Description: Based on interview, the center failed to warm breast milk as the regulations require.
1. The inspector interviewed the infant teacher about the center's procedures for warming breast milk. The teacher said the breast milk was warmed in the microwave.

Plan of Correction: The director discussed the procedures with the staff for warming breast milk during the inspection. In the future, milk, formula or breast milk will not be warmed in the microwave.
