Standard #: 22VAC40-185-570-C Description: Based on a review of records for infants, information for infant's on formula, and interviews with staff, the center has not maintained records for children on formula that includes the brand of formula and each child's feeding schedule. Evidence: The records for infants did not include parent's writing the brand of commercial formula and how often to feed bottles to their child. Staff indicated obtained verbal instructions from parents for feeding schedules and wrote down feeding due times for the day on the chalkboard. Plan of Correction: The director will create a form for parents to write the brand of formula and feeding schedule for their child. The infant teachers will make sure the written information is maintained for each child who is fed formula.
