Technical Assistance:
We discussed the requirements for records for infants on formula. The risk assessment for a violation was provided.

We reviewed requirements for resilient surfacing, emergency procedures maps, pull-up diaper disposal procedures, and storage of hazardous substances.

Comments:
Thank you for your assistance during the unannounced monitoring inspection conducted from 11:00 AM to 3:40 PM. Today, there were thirty children, ages infancy through four years, with seven staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, posted information, agency reports, injury records, emergency supplies and drill records, five staff records, staff training, staff qualifications, five records for children, nutrition, menu, diapering procedures, infant care and records, indoor and outdoor areas. The children have been provided regular outdoor experiences. A variety of age appropriate materials and equipment have been accessible to children. The children have had learning opportunities for fire safety week. The teachers were observed providing frequent, positive interactions with the children in their care.

Description: Based on a review of records for infants, information for infant's on formula, and interviews with staff, the center has not maintained records for children on formula that includes the brand of formula and each child's feeding schedule. Evidence: The records for infants did not include parent's writing the brand of commercial formula and how often to feed bottles to their child. Staff indicated obtained verbal instructions from parents for feeding schedules and wrote down feeding due times for the day on the chalkboard.

Plan of Correction: The director will create a form for parents to write the brand of formula and feeding schedule for their child. The infant teachers will make sure the written information is maintained for each child who is fed formula.
